Apicoectomy

What is an Apicoectomy?

Root canal treatment has a fairly high success rate of 85%. The chances of failure of root canal treatment can be multiple, starting from excess filling, deficient filling, aberrant anatomies of the tooth or surrounding structures, or the size and type of pathology at the time of initial treatment. In these instances, rather than the tooth being condemned for extraction, there is an available treatment option that falls in a category of procedures called surgical endodontics. The procedure is called apicoectomy and is usually performed by our highly experienced oral surgeon in Dubai.

An apicoectomy is a minor surgery which is also known as root end surgery because it involves the surgery of the root end or “apex,” of the tooth. The procedure involves accessing the problem area and removing any infected tissue. The surgeon might also decide to remove a few millimetres from the root tip in order to create a thorough sterile area. Special materials like MTA are used to seal the reverse end of the tooth in order to prevent future leakages and eliminate any further chance of reinfection. This is a painless procedure done under local anaesthesia and the site of the procedure heals within two to three days. Patients are often prescribed antibiotics and analgesics along with standard post-surgery care. The purpose of the treatment is to eliminate the need for extraction and preserve the function of the natural tooth.

What are the Steps Involved?

The Apicoectomy procedure involves the following steps:

Local Anaesthesia: You will be given local anaesthesia to numb the affected area.

Surgery: Once the area has been effectively numbed, the surgeon accesses the site involved. The infected tissue around the root is cleaned out using a procedure called curettage. The surgeon evaluates the need for the removal of the root tip depending on the severity of the case. If it is removed, it is combined by placing a seal on the reverse end of the tooth. In cases of larger bony defects, bone grafting procedures also might be included.

Suturing: The surgeon closes the access to the site with sutures.

Post-Apicoectomy and Recovery: There is bound to be some minor discomfort and swelling post the procedure. You will be able to resume your regular routine within a couple of days. We will also prescribe antibiotics to keep away an existing infection or ward off any postoperative infection. You will need to be careful while flossing and brushing near the area where the surgery has been performed while the stitches are still there. Within a week, the stitches are removed.

What can I do to ease pain after apicoectomy surgery?

We recommend you follow these steps after your apicoectomy procedure.

  • Apply ice to the swollen area for 20 minutes. Repeat after the next 20 minutes till your pain subsides.
  • You can use over-the-counter pain medication to help with pain relief and swelling.
  • Rinse your mouth but not vigorously.
  • Do not brush your teeth nearing the surgery site.
  • Eat only soft food. Stay away from hot, spicy, hard and crunchy food as it can easily disturb the affected area.

What can I expect during the recovery period after the apicoectomy?

You can go back to your normal routine after the first 24 hours. There might be slight swelling and discomfort but it will subside in a day or two. Do follow the dentist’s instructions on brushing and diet. If your pain does not subside with the prescribed painkillers, you may contact your dentist.

What is the apicoectomy success rate?

Apical surgery is an extremely reliable way to preserve those teeth that have been affected by an infection or other problems at the root. A recent study that was conducted in the year 2020 has pegged the success rate of about 80% of the cases worldwide. In most cases, the patients showed excellent results which lasted them for about 5 years and in 75% of the cases for about 10-13 years.

How long will the procedure take?

The entire surgery usually takes about 30 to 90 minutes. However, the time can vary according to the tooth that needs treatment and also according to the intricacy of the root structure.
